This form is for individuals attending The Post Care Collective's in-person events.
The Terms of Use & Community Guidelines is separate from the support-group registration process and is not part of the support-group registration steps.
Support Group Consent Form
This form is specifically for individuals participating in a Post Care Collective support group.
Please review and complete the Support Group Consent Form before continuing with the support-group registration process.
Group Interest Form
The Group Interest Form is a separate form used to express your interest in joining a Post Care Collective support group.
This form is separate from both the Support Group Consent Form and the Terms of Use & Community Guidelines.
